CMN Hospitals Impact Award Nominations – CHW 2021
We need your help to recognize an outstanding provider or caregiver during Children’s Hospitals Week. This year’s award recipient will be the first in this special category of care. We are asking US and Canada hospitals to consider nominating a doctor, nurse, caregiver, or department/team that provides exceptional care and is supported by CMN Hospitals funds. You can fill out this form to make a nomination. Nominations will be accepted until March 5.
