Thank you to everyone who has returned their 2019 signed membership amendment and base fee payment.
As a reminder, the signed amendment and initial base fee payment are due January 1, 2019. Please send a scanned copy of the amendment to Amanda Thomason at athomason@cmnhospitals.org. Payments can be mailed to CMN Hospitals at 205 West 700 South, Salt Lake City, UT 84101.
